Access to health care is a right,
not a privilege

The Health Care for the Homeless (HCH) Program of Manchester at Catholic Medical Center, is made possible through generous support from the Manchester Health Department and Catholic Medical Center.

HCH Manchester conducts a primary care clinic at three locations:

All persons who are experiencing homelessness, or who are at risk of being homeless, are welcomed. No one is turned away due to an inability to pay.

Clinic services include:

Primary medical care
Mental health care
Addiction counseling
Nurse case management
Health education
Social services
Assistance with food stamps, Medicaid and disability applications
Dental care and eye care are also available on a limited basis
Patients are welcome to walk-in for care at locations listed below

An innovative delivery

Connecting Care

Manchester Mobile Health Care connects comprehensive healthcare services on-site to individuals throughout our community. It brings all of the functions of a doctor’s office to the streets, allowing providers to conduct checkups, draw bloodwork, give vaccinations and provide counseling.

DIAGNOSIS, TREATMENT AND TECHNOLOGIES

CLINIC TEAM

The HCH clinic team includes physicians, nurse practitioners, a psychiatric nurse practitioner, nurses, a social worker and a program assistant.

STREET OUTREACH

HCH Manchester also employs a street outreach nurse who meets people at the Homeless Services Center, smaller shelters in town, and at other locations to assist people who would like to visit or enroll in the HCH clinic for care.

HEALTH EDUCATION

HCH Manchester employs a Health Educator who conducts classes at clinic, at Families In Transition Adult Emergency Shelter and at other locations in town. HCH Manchester collaborates with specialty health care providers and social service agencies statewide to insure comprehensive care for all patients.

Hypertension is a leading cause of heart attacks, so the team at HCH used the grant to improve the way they measure blood pressure and help patients with hypertension keep it under control.
