Heath Care for the Homeless (HCH) Manchester offers a comprehensive care team to address patients' medical, behavioral and social needs.
Services at the Families In Transition Adult Emergency Shelter are available to patients ages 18 and older. No appointment necessary.
Services Include:
- Primary medical care
- Mental health care
- Substance use services
- Medication assisted treatment
- Nurse case management
- Health education
- Community resources
- Assistance with food stamps, Medicaid and disability applications
- Dental care and eye care are also available on a limited basis
