How can I find out if someone is a patient at CMC?
We cannot provide patient information online. To inquire about a patient at CMC, please call: 603.668.3545.  Please keep in mind if someone chooses to be a “privacy patient” our staff is unable to share any information about that patient.
I have a condition I would like medical advice for. Can you provide advice via e-mail?
Catholic Medical Center does not provide medical advice via e-mail. We encourage you to discuss any medical concerns with your primary care provider. If your condition is an emergency please call 911.

I have a question about my prescription or need help refilling my prescription?
Please contact the provider listed on the current prescription label with any questions and to request refills. For an alphabetical list of CMC's practices and contact information telephone information, please scroll below these frequently asked questions.
Can I get my prescription delivered before I am discharged from the hospital?
Yes, for your convenience CMC and Rite Aid partner to provide a Meds-to-Beds service that includes FREE delivery of medications to your hospital room (9 AM–8 PM), the Discharge Hospitality Lounge or to your home Mon-Fri (9 AM–12 PM). Please ask your nurse about the Meds-to-Beds form.
How can I find a doctor?

How do I find out if a practice is accepting NEW PATIENTS?

How can I submit a complaint or compliment?
We greatly value your feedback. If you wish to convey a compliment or express a concern regarding your care at Catholic Medical Center, please ask to speak with the Department Director.

How can I request copies of my MEDICAL RECORDS?
Due to privacy practices, medical records cannot be requested online. You must complete and submit an Authorization to Release form to the CMC Health Information Management/Medical Records department. This form, and additional information about how to request copies of your medical record can be found by visiting our Medical Records page. You may also call the office at 603.663.6516 (M-F).
How do I obtain a copy of my x-ray/CT scans/MRI?
Contact our Radiology Imaging Department at 603.663.6449 or register for CMC's Patient Portal and download an Authorization Release form. 
How do I cancel or reschedule an appointment?
Please call the practice in which you have a scheduled appointment with to cancel or reschedule your appointment. A telephone directory is listed below. If you are unsure which practice your provider is associated with, please use the CMC Find a Doctor tool to view this information.
What are the hours for the lab?
CMC has several Laboratory Patient Service Centers in Manchester and the surrounding communities. Please click here to view locations and location-specific information.

How can I get answers to questions about my bill?
CMC's Patient Financial Services representatives are available Mon–Fri 8 AM–4 PM to answer any questions you may have about your hospital bill. Representatives may be reached at 603.663.6922.   
How can I find out how much it will cost for services at CMC?
Cost estimates for medical procedures are complex, but the staff at CMC is committed to helping you through the process. How can I obtain a birth or death certificate?
CMC does not provide copies of birth or death certificates. Please contact the State of NH—Vital Records at 603.271.4650.  
I misplaced something while at CMC, how do I contact the lost & found?
