Foundation for Healthy Communities Honors Catholic Medical Center for Patient Engagement

(From The Foundation for Healthy Communities/ September 27, 2016) - The Foundation for Healthy Communities recently honored Catholic Medical Center with its Noah Lord Award for Patient & Family Engagement during the Foundation’s annual meeting at the Omni Mount Washington Resort in Bretton Woods. Accepting the award on behalf of Catholic Medical Center was Barbara McGuire who currently serves as an Advisor on the hospital’s Patient and Family Advisory Council.
The Noah Lord Patient & Family Engagement Award recognizes the innovative work being done by Patient and Family Advisory Councils to better engage patients and families with Patient Family Advisors to improve patient safety, quality improvement and the patient experience in a NH hospital system. The award celebrates efforts to create change through improving communication between patients, families and the medical team, and is designed to facilitate the sharing of success stories across the state.

The Foundation for Healthy Communities presented the 2016 Noah Lord Patient & Family Engagement Award to Catholic Medical Center for their project titled, The Voice of the Patient: Improving the Patient Experience by Listening to the Voice of the Patient.

“By engaging the patient at the bedside to address any concerns or challenges they might be facing, Catholic Medical Center is able to immediately impact that patient’s experience, improving the communication between patient and provider, as well as the care delivered,” stated Shawn LaFrance, Executive Director of the Foundation for Healthy Communities. “Recognizing those efforts and sharing their success is the essence of this award, and we look forward to celebrating the future successes of others,” he continued.

The innovative program was initiated by members of the hospital’s Patient & Family Advisory Council to connect trained Patient Family Advisors with patients in the hospital in a structured conversation to assess that patient’s current in-hospital experience. The real-time information or feedback regarding the patient’s experience is relayed directly to the hospital staff on that patient care unit by the Patient Advisor with the goal of addressing current needs and providing that patient with a better patient experience.

“It is an honor for CMC to receive this award, as we strive to ensure that our patients and families are at the center of all we do,” stated Karen McLaughlin, who is a Patient Liaison at Catholic Medical Center. McLaughlin continued, “What was unique about this project is that it was 100% patient driven; from inception to implementation. The program was created by former patients to serve current patients; exemplifying patient and family engagement.
