NH Value Care—Accountable Care Organization (ACO) 
Overview
NH Value Care, LLC is a limited liability corporation that was set up in 2018 for the express purpose of participating as an Accountable Care Organization (ACO) in the Medicare Shared Savings Program in partnership with other healthcare organizations. NH Value Care is a joint venture between the following hospitals: GraniteOne Health, including Catholic Medical Center, Huggins Hospital and Monadnock Community Hospital as well as certain members of North Country Community Care Organization, LLC, including Androscoggin Valley Hospital, Littleton Regional Hospital, Upper Connecticut Valley Hospital and Weeks Medical Center. Collectively these organizations employ over providers and provide services to approximately 20,000 Medicare beneficiaries attributed to the ACO. 

NH Value Care, LLC has not participated in the Medicare Shared Savings Program as an entity. The Medicare Shared Savings Program is a Centers for Medicare & Medicaid Services (CMS) program enabling providers to form ACOs and establishes accountability for the quality and total cost for the ACO's attributed Medicare fee-for-service population. The program offers several "risk tracks" that feature varying levels of financial risk.
