CMC Partners with Easterseals NH to Support Veterans, Service Members, and Their Families

An Easterseals MVS Care Coordinator will be on site at CMC three days a week to work with veterans, military members and their families

Catholic Medical Center (CMC) is proud to announce it is partnering with Easterseals NH Military Veterans Services (MVS) on a first-of-its-kind, hospital-based Care Coordination program to serve patients and employees who are veterans, service members, and family members.

An Easterseals MVS Care Coordinator will be on site at CMC three days a week to work with veterans, military members, and their families. Services available through this program include: resource referral, substance use disorder treatment and wrap-around care, general care coordination, housing and homelessness, employment, women’s issues, military sexual trauma, aging and senior issues, discharge planning, and care transition. Referrals may also be available to health care, benefits, legal issues, mental health services, counseling, and emotional support resources for individuals, couples, and children.

Additionally, emergency financial assistance may be provided through Veterans Count, a fundraising program of Easterseals that provides critical and timely financial assistance and services to veterans, service members and their families.

These free, confidential, and local services will be provided to veterans of any era regardless of VA eligibility, discharge status, length of service, or active duty status. The program is being in implemented at CMC and at Elliot Health System, also in Manchester.
