    Amoskeag Primary Care is located at 1650 Elm Street,Suite 302 in Manchester, NH
    • From points north: Head south on I-93 S. Continue onto I-293 S. Take exit 6 for Amoskeag Street toward Goffstown Road. Keep right at the fork to continue toward Amoskeag Street.  Slight right onto Amoskeag Street (signs for US-3/Canal Street/Elm Street/Southern NH University/Une Manchester). Continue onto W. Salmon St. Turn right onto Elm Street.
    • From points south/east: Take NH-101 W. Take the exit onto I-93 N toward Concord. Take exit 9S to merge onto NH-28 S/US-3 S/Hooksett Road toward Manchester. Turn right onto Webster Street.  Turn left onto Elm Street.
    • From Nashua and other points south: Take the Frederick E Everett Turnpike N ramp to Manchester. Merge onto Frederick E Everett Turnpike. Merge onto I-293 N. Take the exit toward Granite Street. Turn right onto Granite Street. Turn left onto Elm St.reet.

  • Family Health & Wellness Center at Bedford
    Family Health & Wellness Center at Bedford is located at 188 RT 101 in Bedford, NH.
    • From points south: I-93 North or Route 3 North to I-293 North to Route 101 West. Follow 101 West toward Milford/Keene. Family Health & Wellness Center at Bedford is approximately 1.5 miles up on your left. 
    • From points north: I-93 South to I-293 South to Route 101 West. Follow 101 West toward Milford/Keene. Family Health & Wellness Center at Bedford is approximately 1.5 miles up on your left.

  • Family Physicians of Manchester
    Family Physicians of Manchester is located at 57 Webster Street in Manchester, NH. Patient parking lot is behind the building. 
    • From points north: I-93 to 293 South to exit 6 (Amoskeag Bridge). Bear right off the exit onto traffic circle. Bear right at the Amoskeag Inn through the light onto the Amoskeag Bridge. At next light turn left onto Elm Street. Travel ½ mile to next set of lights; turn right onto Webster Street. Travel ¼ mile and turn right at blinking light onto Chestnut Street; turn left into second parking lot. 
    • From points south/east: Travel 101 East to I-93 North. Go exit 9S. Bear right off exit onto Daniel Webster Highway. Travel straight on Daniel Webster Highway through three sets of lights (about 2 miles). At fourth light, bear right onto Webster Street (Dunkin Donuts on the right). Travel straight on Webster Street through two sets of lights. Turn left at blinking yellow light onto Chestnut Street; turn left into second parking lot. 
    • From Nashua and other points south: Travel Route 3 North (Everett Turnpike) to Manchester. Take Exit 6 (Amoskeag Bridge). Bear right off exit onto traffic circle. Bear right at the Amoskeag Inn through the light onto the Amoskeag Bridge. At next light turn left onto Elm Street. Travel ½ mile to next set of lights; turn right onto Webster Street. Travel ¼ mile and turn right at blinking light onto Chestnut Street; turn left into second parking lot.

  • Goffstown Primary Care
    Goffstown Primary Care is located at 17A Tatro Drive, Suite 201, Goffstown NH.
    • From points north: I-93 South to 293 south to 101 West to Route 114 toward Goffstown. At last set of lights, turn left onto Mast Road (114A). After approximately 2.5 miles, Tatro Drive will be on your left, immediately after the Shell station. 
    • From points south: I-93 North to 293 North to 101 West to Route 114 toward Goffstown. At last set of lights, turn left onto Mast Road (114A). After approximately 2.5 miles, Tatro Drive will be on your left, immediately after the Shell station. 
    • From points east: Route 101 West to Route 114 toward Goffstown. At last set of lights, turn left onto Mast Road (114A). After approximately 2.5 miles, Tatro Drive will be on your left, immediately after the Shell station. 
    • From points west: Route 101 East to Route 114 toward Goffstown. At last set of lights, turn left onto Mast Road (114A). After approximately 2.5 miles, Tatro Drive will be on your left, immediately after the Shell station.

    Granite State Internal Medicine is located at 188 Route 101 Bedford, NH.
    • From points south: I-93 North or Route 3 North to I-293 North to Route 101 West. Follow 101 West toward Milford/Keene. Family Health & Wellness Center at Bedford is approximately 1.5 miles up on your left. 
    • From points north: I-93 South to I-293 South to Route 101 West. Follow 101 West toward Milford/Keene. Family Health & Wellness Center at Bedford is approximately 1.5 miles up on your left.

  • Hooksett Primary Care
    Hooksett Primary Care is located in Riverside Park Plaza at 11 Kimball Drive, Hooksett, NH.
    • From Manchester I-93: Take I-93 North to Exit 10. At the end of the exit is a traffic light. At the light, take a left onto Route 3A. Follow this under the bridge and through the lights until you see Riverside Plaza on your left. Take your next left onto Goonan Drive (Supreme Gas Express is on your right) and then an immediate left onto Kimball Drive which takes you into the plaza. 
    • From Nashua Route 3: Take Route 3 to I-293 North to exit 7. At the end of the ramp, merge onto Route 3A heading to Hooksett (also known as West River Road) and follow approximately 3 miles until you see Riverside Plaza. Turn right onto Goonan Drive and an immediate left onto Kimball Drive which takes you into the plaza. 
    • From Concord I-93: Take 93 South for approximately 13 miles. Take exit 10 and take a left onto Route 3A. Follow this under the bridge and through the lights until you see Riverside Plaza on your left. Take your next left onto Goonan Drive (Supreme Gas Express is on your right) and then an immediate left onto Kimball Drive which takes your into the plaza.

    The Medical Group of Manchester is located in Dartmouth Commons at 775 South Main Street in Manchester, NH.
    • From points north: I-93 to 293 South to exit 4. Take a left on to Second Street. At the first set of lights, take a right onto South Main Street. Dartmouth Commons is immediately on your left. 
    • From points south: I-93 North to 293 North to Exit 4. Stay left off exit and at light take left onto Second Street. At the first set of lights, take a right onto South Main Street. Dartmouth Commons is immediately on your left. 
    • From points east: Route 101 West to Route 3 Exit in Bedford. Stay in the left lane; at light, turn left onto Route 3. At fourth light, bear left onto South Main Street. Dartmouth Commons is immediately on your left. 
    • From points west: Route 101 East to Route 114 to Exit 3-Meetinghouse Road/South River Road. At the end of the exit, take a left onto Meetinghouse Road. At light, take a left onto South River Road/Route 3. Follow Route 3 to a fork/light. At light, bear left onto South Main Street. Dartmouth Commons is immediately on your left.

    • From points north: I-93 to 293 South to Exit 4. Take a left on to Second Street. At the first set of lights, take a right onto South Main Street. Dartmouth Commons is immediately on your left. 
    • From points south: I-93 North to 293 North to Exit 4. Stay left on exit and at light take left onto Second Street. At the first set of lights, take a right onto South Main Street. Dartmouth Commons is immediately on your left. 
    • From points east: Route 101 West to Route 3 Exit in Bedford. Stay in the left lane; at light, turn left onto Route 3. At fourth light, bear left onto South Main Street. Dartmouth Commons is immediately on your left. 
    • From points west: Route 101 East to Route 114 to Exit 3-Meetinghouse Road/South River Road. At the end of the exit, take a left onto Meetinghouse Road. At light, take a left onto South River Road/Route 3. Follow Route 3 to a fork/light. At light, bear left onto South Main Street. Dartmouth Commons is immediately on your left.

    Willowbend Family Practice is located at 5 Washington Place in Bedford, NH. Our office is located in the same building as CMC Urgent Care at Bedford.
    • From points north: I-93 South to 293 South to Exit 4 (Queen City Bridge). At end of ramp, take a left onto Second St. Go past Hannaford on the right. After Bedford Commons, take a left at the Washington Place light to turn into Bedford Medical Park. 
    • From points south: I-93 North to 101 West/293 North. Stay on 101 West at the split. Take the US-3 North exit (Kilton Road). Take left at third light onto South River Road and take a right at the Washington Place light to turn into Bedford Medical Park.

        CMC receives a 3 star rating by the Society of Thoracic Surgeons
        The Society of Thoracic Surgeons has developed a comprehensive rating system for the quality of cardiac surgery among hospitals across the country. Approximately 12-15% of hospitals received the 3 star rating which denotes the highest category of care. Since January 2010 the cardiac surgery performance of our hospital was found to lie in the highest quality tier, thereby receiving an STS 3 star rating.
