Medical Records

Obtaining your Catholic Medical Center records should be easy and we are happy to assist. To maintain confidentiality and privacy, please choose one of the following options.

  1. -Download "Authorization to Release" here. Complete and fax to 603.663.6531
  1. -Complete an Authorization to Release form and deliver it or leave in the available locked box. 

    -Office hours: Mon–Fri 8 AM–4:30 PM. 



Please note that Health Information Management/Medical Records office will be relocating to 195 McGregor Street, Suite 110, during the week of 9/11-9/15. During this time, the department will have limited operation and will mail records to patients homes. Completed request forms can faxed to 603.663.6531 or dropped off at 195 McGregor Street, 1st Floor (the large, brick mill building across the street from the hospital–use the South entrance).
