Community Health Services
Caring for Our Community Caring for those in need has been a longstanding mission of Catholic Medical Center. We reach out to members of the community through programs and services that offer health assessment, education, follow-up, referrals and resources to help individuals access the health information and care they need to live healthy, productive lives.
Modifying lifestyle behaviors and taking an active role in managing your health can reduce the consequences of chronic disease and greatly improve your quality of life. Health risk behaviors such as lack of physical activity, poor nutrition, tobacco use, and excessive alcohol consumption are responsible for much of the illness, suffering, and early deaths related to chronic diseases.
Community Health Services offers a variety of health self-management programs that can help you live a healthier life. To participate in these free programs, contact us directly or you may be referred by your physician.
