The Social Work/Case Management Department at Catholic Medical Center is pleased to offer services to patients of Catholic Medical Center, and their families. In addition to the inpatient setting, our services are available throughout outpatient departments, physician practices and members of the Catholic Medical Center community. Our team includes professional Social Workers and RN/Case Managers.

Social Work/Case Management staff work collaboratively with members of the medical staff and interdisciplinary team, including patients and their families, to promote coordination of care. This includes transition from the hospital to home and/or to the most appropriate care setting.

Illness and disability can present many challenges to individuals and their families. The Social Work/Case Management team is available to assist you in sorting through the many options available to promote your individual social, financial and psychological wellness as part of your health care.


Advance Care Planning
Educational information and assistance with executing Advance Care Directives is available through the Social Work/Case Management Department. We are pleased to offer our patients and members of the community the opportunity to take a proactive approach in planning for future health care needs. Advance Care Directive documents include the Durable Power of Attorney for Health Care and Living Will.
