You may also want to check with your insurance company regarding the use of specialists; our referral or authorization does not guarantee coverage or payment by your insurance company.
When you call us, please have the following information available:
- Full name
- Date of birth
- Daytime phone number
- Health insurance information

Your regular physician
- Physician to whom you would like the referral, their address and phone number
- Reason for the referral
No retroactive referrals can be made by our physicians for services received without their knowledge.
Please allow 7 to10 days for the processing of referrals.
