Please contact our office to request necessary referrals before scheduling any non-emergency visits with specialists.  You may also want to check with your insurance company regarding the use of specialists; our referral or authorization does not guarantee coverage or payment by your insurance company. 

 

When you call us, please have the following information available:

• Full name
• Date of birth
• Daytime phone number
• Health insurance information
• Your regular physician
• Physician to whom you would like the referral, their address and phone number
• Reason for the referral

 

No retroactive referrals can be made by our physicians for services received without their knowledge.

 

Please allow 7-10 days for the processing of referrals.
