Catholic Medical Center has a deeply rooted history of providing care to those most in need. It is part of our heritage, a part of what we do. Catholic Medical Center reaches out to the community through programs and services that offer assessment, education, follow-up, referral and resources to assist each person with access to health information and care. We are a dedicated department of Community Health Services; we reach out to those most in need through several of our services such as the Parish Nurse Program, Poisson Dental Facility, Health Care for the Homeless, Prime Time and many others.
Spotlight on Chronic Disease Management Programs
Almost half of the people in the United States have at least one chronic condition and many have multiple. More than 80% of health care costs are expended on the care of patients with chronic diseases. In an effort to support the patient and the health care provider, Community Health Services will be offering a variety of chronic disease self-management programs for the community. Participants may be self-referred or referred by their physicians to these free programs.
