Community Health Services
Catholic Medical Center has a deeply rooted history of providing care to those most in need. It is part of our heritage, a part of what we do. Catholic Medical Center reaches out to the community through programs and services that offer assessment, education, follow-up, referral and resources to assist each person with access to health information and care. We are a dedicated department of Community Health Services; we reach out to those most in need through several of our services such as the Parish Nurse Program, Poisson Dental Facility, Health Care for the Homeless, Prime Time and many others.
Health Self-Management Programs
Four modifiable health risk behaviors-lack of physical activity, poor nutrition, tobacco use, and excessive alcohol consumption-are responsible for much of the illness, suffering, and early death related to chronic diseases. About one-fourth of people with chronic conditions have one or more daily activity limitations.
Modifying lifestyle behaviors and taking an active role in managing ones’ health, can reduce the consequences of chronic disease and greatly improve the quality of life.
In an effort to support the patient and the healthcare team, Community Health Services offers a variety of health self-management programs for the community. Participants may be self-referred or referred by their physicians to these programs at no charge.
